MSHSL forms are required for all MSA athletes each year. Return forms to the MSA Activities Director prior to the first day of practice.

Forms must be returned to Mr. Hoemann or Ms. Shelstad by the date noted on the registration form.
Fees are due no later than the first day of practice.
You must have a physical on file from the doctor's office that is within the last 3 years. Doctors fill out their own but you can download the one on this page and bring with you if preferred. A yearly examination form is also acceptable.
Activity Forms
Anyone interested in holding an event at MSA must complete this form step-by-step including meeting with the activities director, Mr. Hoemann prior to promoting the event. Please use the form below to ensure all steps have been taken as well.
Anyone inviting dance guests must complete the dance form step-by-step. If forms are not completed and returned by 48 hours prior to dance event, guest will not be granted permission.
